Automated update of schema documentation from specvital-infra.

Generated by tbls - includes:

  • Table details with columns, types, constraints
  • Indexes and unique keys
  • ER diagram (Mermaid)

Triggered by: perf(db): add index for spec reuse query

Cross-analysis spec reuse feature queries by (content_hash, language, model_id) without user_id.
Existing unique index has user_id as first column, causing full table scan for this query pattern.

Added new index following B-Tree leftmost prefix rule to enable index scan.
